PSC Waiver and Guest Fees

Please complete the waiver below for each Guest attending PSC today.

For persons under 18 a parent or guardian must complete this waiver.

Remember: All Guests must attend with a PSC Member who has an active membership in the current season.

Provision Swim Club Release and Waiver of Liability *

I, the undersigned, fully understand that there may be medical risks and risk of injury, disability, illness or death associated with physical exercise, the use of the facility, or the use of equipment within the facility. I also recognize that the Provision Swim Club cannot evaluate me or my families physical abilities and medical limitations as they pertain to participation in programs, to the use of the facilities, or use of the equipment within the facility, or injury caused by or inflicted by the acts or negligence myself or of other participants.

I fully understand that there is no supervision or lifeguard on duty at Provision Swim Club so I and my family are swimming and/or playing sports at our own risk.

I expressly agree to accept and assume all of the risks of bodily injury, sickness, death, or property damage arising out of or related to me and my families use of the facility, its equipment, and my/our participation in Provision Swim Club activities.

I also agree that myself, my family, and all persons undersigned (and their guests) shall abide by the Rules & Regulations of the Provision Swim Club, which may be posted, issued orally, or otherwise published. These rules may be amended at the club’s discretion without notice. Failure to do so may result in the termination of my/our further participation in programs at the PSC facility grounds and/or removal from the property, and also may result in forfeiture of fees.

I understand that Provision Swim Club is not responsible for equipment or personal belongings.
